  1. The cost of registration of a single user copy of the Phone Directory Manager
  2. is $30.00 (US).

  4. If you plan to use copies of the registered version on many PC's throughout
  5. your company,  we now offer a one-time site license for the current version
  6. of the program.  The fee is only $150.00 (US), and lets you distribute the
  7. latest registered version to as many PC's within your particular company
  8. as you wish. We provide one copy of the program with your company name on
  9. the opening screen, and a laser-printed copy of the documentation suitable
  10. for reproduction.

  12. D&B rated companies may FAX us purchase orders, along with this form, and
  13. we will invoice you.  FAX orders to (510)676-1284.  The price is $10.00
  14. additional ($40.00 for single copies and $160.00 for site license), and is
  15. subject to credit acceptance.
